PROCEDURE FOR ADMISSION OF NEW & RETURNING STUDENTS:

1. Proceed to the Registrar / Admission’s Office for inquiry
2.  Submit the following requirements for processing: 
 

Birth Certificate (Original & 2 pcs. Photocopy)

Photocopy of the latest report card (1pc.) 
Latest Medical Certificate 
2pcs 1”x1” colored pictures with blue or white background
Brown Plastic envelope (1 pc, Long size)
National Career Assessment Exam (NCAE) result (For Senior High School only; Photocopy) 
Recommendation letter- extended until releasing of test result

Good Moral Certificate – extended until releasing of test result

For Applicants from Foreign Schools: Junior High school Applicants from foreign schools shall take Filipino 7 & AP 7 (and additional Filipino II to a particular level – Grade 9) within the school year as required by DepEd and shall be reflected on their form 137. 
3. Accomplish the admission forms and submit them at Window B or C for final screening and validation of requirements. (Routing form is attached to the application form to be filled up by respected offices for verification)
4.

Proceed to the Clinic Department for verification of health information record.

Note: Applicants with medical condition indicated in the admission/application form will be subject for further verification before being allowed to take the entrance test. 

5. Proceed to the Guidance Department for verification of the Student Inventory Record (S.I.R ) / Behavior Monitoring Assessment Form.(Applicants with behavior problem/s indicated in the form will be subject for deliberation before being allowed to take the entrance test. 
6. Proceed to the Accounting Office to pay the required entrance test fee at Window D or E.(for Preschool, Grade School and Junior High School only)
7. Return the admission/ routing form to the Registrar’s office for scheduling of Interview (by the Guidance Facilitator), entrance exam & issuance of test permit. 
Note: 
Interview with the applicant by the guidance facilitator is conducted prior to the day of examination. (For Grade 4 to Grade 12 applicants only)
Short Interview with the applicant accompanied by parent/s is required prior the day of examination (Preschool to Grade 3 applicants only). 
8.  Secure the examination permit (date, time, and location of examination are indicated in the permit) from the Registrar’s Office (WINDOW B). 
9.

Take the entrance examination on the scheduled date. All applicants must present their exam permits on the date of exam.

Note: Test results will not be released unless recommendation letter & good moral certificates are submitted

10. On the day of the examination, the student must proceed to the Guidance office for testing. Student must bring and present the exam permit to the Guidance Facilitator who is authorized to conduct the testing.
11.  Please inquire at the Registrar’s Office regarding the result of the test (whether they passed, failed or are for reassessment – PS to Grade 1).
