Admission Requirements and Enrollment Procedures

PROCEDURE FOR ADMISSION OF NEW STUDENTS

1. Proceed to the Registrar/Admissions Office for inquiries.
2. Submit the following requirements to the Registrar/Admissions Office.
 

Document Requirements are the following: 


Latest report card (1 pc photocopy)
a. Grades not in numeric form must be provided with numerical equivalent  
b. English translation must be provided for report cards using other languages.

Birth Certificate from National Statistical Office (NSO) (Original and 1 pc photocopy)
2 pcs. 1”X1” ID colored picture with blue or white Background
1 pc Long Size Plastic  Envelope
National Career Assessment Exam (NCAE) result (For Senior High School only; Photocopy)
Recommendation Form from Guidance Counselor or Principal of previous school (For Kindergarten to Grade 12 applicants).

Forms are available at the Registrar’s Office and downloadable from the school website. www.statefields.edu.ph (Filled-out Recommendation Form should be submitted before the release of the Entrance Test result)

3. Accomplish the admission forms and submit it at Window B or C for final screening and validation of requirements.
4. Proceed to the Clinic Department for verification of health information record.
5. Proceed to the Accounting Office to pay the required entrance test fee at Window D or E.(for Preschool, Grade School and Junior High School only)
6. Submit the accomplished recommendation form and take the entrance examination on the scheduled date. All applicants must present their exam permits on the date of exam. Test results will not be released unless all requirements are already submitted.
7. Secure the examination permit (date, time, and location of examination is indicated in the permit) from the Registrar’s Office (WINDOW B). 
8. On the day of the examination, the student must proceed to the Guidance Department for testing. Student must bring and present the exam permit to the Guidance Facilitator, who is authorized to conduct the testing.
9. Please inquire at the Registrar’s Office regarding the result of the test (whether  they passed, failed or for re-assessment).
 
Admission Requirements and Enrollment Procedures

STUDENT  ENROLLMENT  PROCEDURE

A. New Students Including Returnees
1. Qualified students must proceed to the Enrollment Area for enrollment process.
  Note: Admission Committee will interview qualified High Students first before proceeding to enrollment process.
2. Parents/student applicants should:
 
a. Present original report card duly signed by former school principal.
b. Submit financial clearance form from the last school attended.
c. Get the pre-printed Enrollment Form from the Admissions/Registrar’s Office. Check and sign the necessary information. After completion of the forms, return it to the Enrollment Area then have it stamped “For Assessment”.
d. Get a number for Assessment.
3. Proceed to Assessment Area for estimation of applicable tuition and miscellaneous fees. Please wait for your number to be called or flashed on the queuing system. 
4. Proceed to Cashier and get a number. Once your number is called or flashed on the queuing system, kindly proceed to the designated Cashier’s window (Window E or D) and pay the applicable “Upon Enrollment Fees”. Payments shall be based on the mode of payments chosen. No Partial Payment will be accepted.
5. Go back to the Registrar’s Office to indicate your preferred schedule. (FOR PRESCHOOL ONLY: FIRST-COME-FIRST SERVED.)
6. Proceed to the Admissions Office for I.D. picture taking for NEW STUDENTS ONLY. (PS, GS, JHS and SHS)
7. Purchase books, school supplies and uniforms in Room 110, 109 and 106 respectively. We are accepting CASH, CREDIT CARD, and DEBIT CARD (ATM) payments for books, uniforms, notebooks and diaries. Pls. pay directly to the Bookstore cashier in Room 107.
 
B. For Old Student

1.

Proceed to the Enrollment Area.  Parents/student should:
a. Present original / previous report card for evaluation and / or authentication / cancellation purposes.
b. Get the pre-printed Enrollment Form. Check and sign the necessary information. After completion of the forms, return it to the Enrollment Area then have it stamped “For assessment”.
c. Get a number for assessment.

2. Proceed to Clinic Department for verification of health information record.
