STUDENT ENROLLMENT PROCEDURE

A. For New & Returning Students:
 
1. 

Qualified students must proceed to the Enrollment Area for the enrollment process

2.  Parents/student applicants should:
 
a.  Present original report card with LRN number and duly signed by former school principal- if not submitted yet & (Note: If the applicant has no original report card, enrollment will be on hold)
b.  Latest medical certificate- if not submitted yet
c.  Get the pre-printed enrollment forms from the Enrollment Area. Check and sign the necessary information. After completion of the forms, return them to the Enrollment Area.
d.  Get a number for Assessment.
3.  Proceed to the Clinic Department for verification of health information record. (Clinic should review carefully the health record of those with ON HOLD status during admission for confirmation prior to enrolment)
4.  Proceed to Assessment Area for estimation of applicable tuition and miscellaneous fees. Please wait for your number to be called or flashed on the queuing system.
5.  Proceed to Cashier and get a number. Once your number is called or flashed on the queuing system, kindly proceed to the designated Cashier’s window (Window E or D) and pay the applicable “Upon Enrollment Fees”. Payments shall be based on the mode of payments chosen. No Partial Payment will be accepted
6.  Go back to the Registrar’s Office to indicate your preferred schedule. (FOR PRESCHOOL ONLY: FIRST-COMEFIRST SERVED.)
7.  Proceed to the Admissions Office for I.D. picture taking, for NEW STUDENTS ONLY. (PS, GS, JHS and SHS)
8.  Purchase books, school supplies and uniforms in Room ___, and ____ respectively. We are accepting CASH,CREDIT CARD, and DEBIT CARD (ATM) payments for books, uniforms, notebooks and diaries. Pls. pay directly to the Bookstore cashier in Room 107.
B. For Old Student
 
1.  Proceed to the Enrollment Area. Parents/student should:
a.  Present original / previous report card for evaluation and / or authentication / cancellation purposes.
b.  Get the pre-printed Enrollment Form. Check and sign the necessary information. After completion,return them to the Enrollment Area.
c.  Get a number for assessment.
2.  Proceed to the Clinic Department for verification/ re-validation of health information record.Note: Old students with medical condition (from the previous year) shall submit first the latest medical certificate/doctor’s recommendation prior re- admission/re-enrolment
3.  Proceed to the Guidance Department for verification of year end assessment. & Note: Old students under guidance monitoring shall submit all Dev-Ped Assessment and Doctor’s Recommendation to the Guidance Department along with the parent/s interview form PRIOR to their re-admission/re-enrolment. Official list shall be forwarded by the clinic /guidance office to the admissions office and other concerned offices for monitoring.
