Online Admission Requirements

PRESCHOOL : NURSERY 1 AND 2

A. AGE BY AUGUST (with AGE EXTENSION until October 31, 2024)     


  • Nursery 1 - Applicant must be 2.10 to 3.9 years old
  • Nursery 2 - Applicant must be 3.10 to 4.9 years old

B. ACADEMIC AND BEHAVIORAL RECORDS     


  • Filled-out online Admission application forms (Personal Information, Health Information, Student Inventory Record)
    Please see link in the procedures
  • Clearly scanned / Electronic (PDF FORMAT) copy of Birth Certificate, preferably PSA ISSUED
  • Solo Picture following the guidelines which is embedded in the Admissions Portal
  • NOTE: If an applicant is from Foreign Country:
     Applicant must be a Filipino Citizen

C. MEDICAL RECORDS     


  • Additional medical records may be required from applicants with medical conditions indicated in the online admission application form. The Clinic Department and the school's attending physician will evaluate the submitted medical record/s prior to proceeding with the next step

D. INTERVIEW and ENTRANCE EXAMINATION     


  • Interview and examination will be conducted onsite.
PRESCHOOL : KINDERGARTEN

A. AGE BY AUGUST (with AGE EXTENSION until October 31, 2024)     


  • Applicant must be 4.10 to 5.9 years old

B. ACADEMIC AND BEHAVIORAL RECORDS     


  • Filled-out online Admission application forms (Personal Information, Health Information, Student Inventory Record)

  • Clearly scanned / Electronic (PDF FORMAT) copy of the following:
    - Birth Certificate, preferably PSA ISSUED
    - Latest Report Card (Form 138) or Transcript of Record (Form 137), those with previous schooling only
     Grades not in numeric form must be provided with numerical equivalent
  • Solo Picture following the guidelines which is embedded in the Admissions Portal
  • Recommendation letter (if with prior schooling only)

    To the Parents & Guardians, please download the Recommendation/Good moral form then submit to your child’s current school. The school will be the one to send the filled-out form to the Guidance Office through the email address found in the downloadable form.

ELEMENTARY SCHOOL : GRADE 1 to GRADE 3

A. GRADE 1 - Applicant must be a KINDERGARTEN Completer, with Learner's Reference Number (LRN)

  • Clearly scanned / Electronic (PDF Format) or original copy of the latest Report Card (either from 1st, 2nd, 3rd quarter grades) or Transcript of record (Form 137) if already available.

     Grades not in numeric form must be provided with numerical equivalent
     No grade lower than 80 in major subjects
    NOTE: If there are grades lower than 80 in major subjects (English, Math & Science), parent/guardian should write a letter addressed to the Principal for approval prior admission

    D. FOR JUNIOR HIGH SCHOOL ESC RECIPIENTS (incoming Grade 8-10) 


    • Please submit an updated ESC Certificate from the previous school.
    SENIOR HIGH SCHOOL

    A. ACADEMIC AND BEHAVIORAL RECORDS

    • Clearly scanned / Electronic (PDF FORMAT) copy of the following:
      - Birth Certificate, preferably PSA ISSUED
      - Original Copy of the Latest Report Card (Form 138 either from 1st, 2nd, or 3rd quarter grades) or Transcript of Records (Form 137) if available
         For Grade 12 transferee/applicant, the submitted Grade 11 Form 138 will be assessed by the Senior High School Vice Principal, in reference to the curricular offerings of the Senior High Department for the current school year. Once approved, the Grade 12 applicant may continue with the admission procedures for new student.
